Under Regulation 288 of the Work Health and Safety (General) Regulations 2022 a registration holder must notify WorkSafe WA if their plant registration document is lost, stolen or destroyed. Regulation 288 also allows the registration holder to apply for a replacement document. The Form 203 - Application for a replacement document process allows registration holders to meet the requirements of regulation 288 while also requesting a replacement of their registration documentation.
Who can apply to replace a plant registration document?
Only the current registration holder may apply to replace a registration document.
What documents can WorkSafe WA replace?
WorkSafe WA can only replace registration documents that have been issued under a Western Australian Work Health and Safety, Mining Safety or Occupational Health and Safety law. WorkSafe WA cannot replace registration documents issued by other States or Territories. To have a document issued by a regulator in another State or Territory replaced you will need to contact that regulator.
Apply for a replacement document
Applications for a replacement document may be made online or in person. While applications will be accepted in person, it is recommended that applications be made online. Applications by email will not be accepted. In order to apply for a replacement document, certain information is required. Online application
You can apply to replace your plant registration document by completing our online application form. This process will require you to provide information and pay the relevant fee. Once submitted, you will automatically be emailed a tax receipt for your payment along with a lodgement confirmation.
Apply in person
The Department of Energy, Mines, Industry Regulation and Safety operates a customer service counter at 303 Seven Oaks Street in Cannington. To lodge an application in person, complete a Form 203 - Application for a replacement registration document and bring it to:

Information required to apply for a replacement document
Show moreIn order to successfully apply for a replacement registration document, the following information will need to be provided:
Registration holder details:
- Type of registration,
- If the registration holder is an individual or a company,
- If an individual:
- The name of the registration holder;
- The business name (if applicable); and
- The applicant's address, email and phone number;
- If a body corporate/ company:
- The company name, as registered with ASIC;
- The designated contact person;
- The business name (if applicable);
- The registered company address, as registered with ASIC; and
- The contact person's email and phone number;
Circumstances in which the registration was lost, stolen or destroyed:
- If the registration lost, stolen or destroyed.
- A brief description of the circumstances in which the registration was lost, stolen or destroyed will need to be provided.
Declaration by registration holder:
- The registration holder will need to declare:
- That the information provided is true and correct; and
- That they are making an application for a replacement registration document in accordance with regulation 288 of the Work Health and Safety (General) Regulations 2022.
