What do I need to lodge a complete request
- complete all the relevant sections of the request form.
- Attach/pay the correct fee. This is payable at the time of lodging the request. Where a certificate has been stolen no fee is payable on provision of a police report number.
Lodging your application
Once you have completed the request form, you can lodge your application in one of two ways:
