These forms are used for requesting a training record and logbook. Use the request form's check list to make sure you satisfy all requirements.

Submitting a completed training record and logbook
After the training record and logbook are finalised, complete the below application form and submit with
- Fit and proper assessment
- Proof of identification
- Completed Restricted Electrical Licence System Training Record and Logbook (purple logbook)
