Application to renew a high risk work assessor accreditation.

Application form

Once you have completed the application form and have all of your supporting documents ready, you can lodge your application in by post or in person.

Payment form

You must also complete the application payment form and attach it to your application.

Applications received without payment cannot be accepted.

Photograph Requirements

Please refer to the Licence card photographs page for details of the standard of identification photograph required to be submitted.

Conditions of accreditation

Accredited assessors must comply with the provisions of the Work Health and Safety Act 2020, Work Health and Safety (General) Regulations 2022, and the conditions of accreditation.

Compliance requirements

Notifications

Accredited assessors are required to notify WorkSafe before any assessment is conducted. Notifications are required to be made using an online form found here: Online forms

Maintenance of experience

Assessors must maintain their competency to assess each class of work they are accredited in. Accreditation will not be renewed if competency is not maintained.
