The Communities Inclusion Connection Team (CICT) is a person-centred, holistic, community-based approach designed to facilitate connection to appropriate mainstream and specialised services for people across Western Australia (WA). Through complex case management, collaborative working approach across community, the team will support individuals and families to access cohesive seamless services that will meet their individual needs so they can lead their best lives.
The team has three core functions:
Communities Inclusion Navigation – initial point of contact, providing responsive, accurate and timely information, signpost, and link people to appropriate services.
Communities Inclusion Connection – community-based, direct support team providing service/system navigation, episodic case management, early intervention, advocacy and planning for individuals with diagnosed disability.
Communities Inclusion Engagement – community-based, directly linking with key stakeholders/community groups to build capacity, promote inclusion, provide community education, identify gaps and barriers to services and develop solutions to address unmet needs.
Key focus will be the provision of early intervention supports to build capacity, reduce the potential impacts that people with disability, their families, and carers experience. Family-led, place-based, community supports will increase equality of opportunity and access to early intervention for people with disability. Supports will be targeted, yet flexible and designed to promote self-efficacy, build natural networks, and prevent people from reaching a crisis.
Who can access CICT?
With a no wrong door approach, anyone within WA can access our team of Community Inclusion Navigators.
People with disability requiring additional support may be eligible to access our Communities Inclusion Connectors if they reside within the Perth Metropolitan Area.
Community members, groups and organisations, not-for-profit services and government agencies can connect in with our Communities Inclusion Engagement Officers.
