Application form

This application form is used to register as a management representative on an Electrical Contractor’s Licence.

• Register as Management Representative for Electrical Contractor (Individual) R106A
• Register as Management Representative for Electrical Contractor (Partnership) R106B
• Register as Management Representative for Electrical Contractor (Company) R106C

By post or in person

If you are submitting your application by post (or in person) you must complete the payment details section of the application form.

Applications received without payment cannot be accepted.
