The declaration form is sent approximately three months before your licence expires. Complete and sign the form, then send it back to the Department of Transport using the pre-paid reply envelope.
Once received, the department makes a decision about your fitness to hold a driver's licence and will advise you of the outcome.
The fitness to drive declaration is open to people with a valid WA’s driver licence but does not apply to:
- new drivers
- people applying for learner’s permits
- drivers who report a new medical condition to the Department of Transport
- taxi, bus and ride share drivers or driving instructors
- drivers aged 80 years or more.
Drivers not eligible for the declaration need to complete a medical assessment certificate with their chosen doctor.
