This Response Form template has been developed for use by State agencies procuring goods and services as part of the Template Redevelopment Project (TRP) Pilot (Phase 1).
The Response Form is used at the contract formation stage of a procurement process. It is issued by the Requesting Agency for completion by the Respondent to document their Offer.
The Response Form template should be read together with the TRP Request for Offers (RFO) template and the underlying conditions documents.
